Please be advised that on June 29, 2016, Dr. Joseph Sofranko is retiring after a long and distinguished career in medicine. We wish him all the best and thank him for his service at Loden Vision Centers.
The physicians, nurses, allied health professionals and staff at our practice will continue to provide the same quality health care that you have received from Dr. Sofranko. If you wish to remain a patient of this practice, there is no action needed on your part.
If, however, you would prefer to seek medical care elsewhere, you are free to do so. You may request a copy of your medical records at our office or you may ask us to transfer them directly to another healthcare provider.
In order to obtain your medical records either personally or to have them sent to a healthcare provider, we will need a signed authorization to transfer records. You may request one while visiting our office or find this on our website at: www.lodenvision.com.
Upon completion of this form, you may return it to our office in person, electronically, or by mail and we will then process your request. Please contact us if you have any questions about your current or future treatment.
